Job Description:
The Case Management Officer is primarily responsible for Child/Adolescent patients, through liaising with various stakeholders, such as family members, doctors, Allied Health staff and external agencies (schools, family service centers etc.). He/She is expected to facilitate the smooth running of Child/Adolescent clinics by performing a range of clinical and administrative duties.
The Challenges:
You will be responsible for the following:
- Collect basic socio-demographic information for all new outpatient patients (and follow-up with patients as required).
- Obtain clinical information from patients via use of scales (ADHD, depression, anxiety rating scales) prior to patient’s first visit in the clinic.
- Contact patients prior to their first visits to confirm their attendances and remind them to attend their first visit and follow up appointments on time.
- Triage cases based on their clinical needs under the supervision of a Psychiatrist.
- Provide psycho-education for patients and their families/caregivers.
- To familiarise with community resources for caregivers and patients.
- Collaborate with Allied Health staff such as Medical Social Workers, Psychologists, Occupational Therapists, and Pharmacists for the management of patient’s assessed needs, including Schools, Family Care Centers, etc.
- Monitor patient’s appointments, investigations, social and rehabilitation plans.
- Ensure that relevant patient education materials are available and are up to date.
- Review key performance indicators timely to promote continuous process improvement.
- Cross cover for other patients who need case management under the care of Psychological Medicine department.
- Perform data management, collating and maintaining of patient database for clinical and research purpose.
- Assist in tracking, maintaining, and reporting of quality clinical indicators and provide statistical report and updates on a regular basis.
- Participate in projects related to case management and clinical pathway.
- Perform other duties and responsibilities, as necessary, in the overall operation of the clinic.
Job Requirements:
Qualification:
- Degree in Nursing, Psychology or relevant discipline.
Experience:
- Relevant experience will be advantageous.
- Good knowledge of adolescent/child care and community skills.
- Computer literacy skills – MS Office (Word / Excel / PowerPoint).
- Ability to work in multi-disciplinary teams and to work independently.
- Excellent interpersonal and communication skills.
