Job Description

To support collective efforts of Offices of Population Health and Care Transformation in the development and implementation of IMH’s strategic objectives in population mental health and transformation of care, aligned to the National Mental Health and Well-being strategy (NMHW). The incumbent will report to Director, OPH & Care Transformation while work closely with Clinical Director, Population Health and Clinical Director, Care Transformation.

Job Responsibilities

  • SPECIFIC
  • Population Health
  • Contribute to planning, development and tracking of IMH’s strategic objectives. Play a key role in ensuring seamless policy-ops coordination to support IMH’s priorities and contribution in population health including formulation of papers/proposals, policy and initiatives.
  • Work closely with key external (e.g. AIC, MOH, NCSS) and internal stakeholders (e.g. departments, services & programmes) to design and drive initiatives to build competency and capacity building of primary care (e.g. GP partnership) and community partners at cluster and national level.

Care Transformation

  • Contribute to efforts in care model development across departments and initiatives as part of clinical services planning and related scope of work (e.g. value-based care, balanced score card). Provide consultancy and support in the governance of care transformative projects, ensuring documentation and follow-up.
  • Provide regular updates and input to senior management to support IMH’s population health and care transformation mandate in meeting objectives under its strategic score-card. Mentor and guide junior colleagues as part of the team effort.
  • Provide secretariat / administrative support in workgroups and committees under population health and care transformation mandate(s) such as the National Mental Health Working Committee.
  • GENERAL
  • To assist in any key collaborative projects as assigned.
  • To cover other team-related portfolio when necessary, as assigned by Reporting Officer.
  • Role model and guide junior colleagues in supporting OPH and CTO driven initiatives.
  • To assist in the preparation of any presentations, minutes and reports as assigned.
  • Any other duties as assigned.

Requirements

Educational And Professional Requirements

  • Bachelor’s Degree and at least 6 years working experience.
  • Post-graduate qualifications an advantage
  • Experienced in project management including planning, implementation and evaluation
  • Well versed in various Microsoft Office applications such as Word, Excel, & Powerpoint
  • Must be proficient with data, budgeting and secretariat work (e.g. committees, workgroups).
  • Work experience in healthcare / hospital/ training/ education industry will be an advantage
