  1. Make sure the webpage is open.

  5. On the left, click Content Blockers.
  6. Make sure Off is chosen in the pop-up menu next to the website.
  7. On the left, click Pop-up Windows.
  8. Make sure Allow is chosen in the pop-up menu next to the website.

Job Description:

The Senior / Executive, Population Health and Integrated Care (PHIC) - Integrated Continuing Care Operations (ICCO), will play an integral role in the team in overseeing and managing a spectrum of Population Health programs. He/she will work with the ICCO team, in partnership with various stakeholders, in both operation and strategic planning aspects of the programs.

Your job roles will include, but not be limited to the following:

  • Consolidate and collate referrals from various sources and flow cases to necessary parties for clinical assessment and intervention where necessary.
  • Regular maintenance of databases to keep an accurate and updated record of cases that are received by the programme.
  • Collate and report data to internal/ external stakeholders (including SingHealth Office of Regional Health, SORH) at regular intervals where needed. Assist to conduct regular programme evaluation.
  • Develop appropriate methodologies and parameters for data collection and cleaning in line with the aspect of programme(s) being analysed
  • Selecting appropriate resources/ systems to gather data efficiently and effectively
  • Provide diagrammatic/ graphical representations of data where necessary
  • Automate and/ or improve manual processes where possible
  • Identify upstream/ downstream impact of upcoming changes to programme/ landscape. Assist to develop new workflows or adjust existing workflows accordingly.
  • Facilitate and support regular meetings including case discussion in multi-disciplinary meetings, administrative meetings, and preparation of minutes.
  • Any other adhoc duties as assigned

Job Requirements:

· Bachelor's Degree preferably in Biomedical Science, Business Analytics, Data Science or healthcare related will be an advantage

· Proficiency in Microsoft Office Applications, particularly Microsoft Excel spreadsheets and application of formulas

· Experience in data extraction, analysis and automation in projects/ internship/ work will be an advantage

· Strong oral and written communication skills
