- Developing and driving the implementation of Kidney Supportive Care clinic for multidisciplinary management of advanced CKD patients (under supportive care).
- Disseminate and implement hospital's policies and procedure; develop, review, and update the team's standard operating procedures (SOPs), protocols, and case management tools.

- Precept newly joined renal coordinators in delivery of specialty care where needed.

- Work alongside CMO to assist with overall coordination of appointments to Kidney Supportive Care Clinic and referral to community partners (Home hospices/ integrated home health etc.)
- Apply understanding of prevention of medico-legal issues in relation to patient care.
- Participate in quality improvement activities.
- Take on additional responsibilities or tasks as assigned by the Head of Division
- A certificate / diploma in Renal Nursing will be advantageous.
- Registered Nurse with at least 3 years of clinical experience.
- Must have at least 5 years prior working experience in a healthcare environment and have assumed some managerial duties before.
- Knowledge of clinical skills and critical thinking in delivery of patients' care will be advantageous.
- Must demonstrate the ability to resolve problems in all the aspects of the implementation of the Project Renalssance including all operational issues, IT issues, clinical related issues etc.
- Must be able to lead a team of Coordinators in the implementation of the programme to deliver all key performance indicators, motivate staff and ensure knowledge retention and succession (if necessary).
- Possess good interpersonal and communication skill.
- Computer literacy skills - MS Office (Word / Excel / PowerPoint).
- Effectively bilingual.
