Job purpose

The jobholder is primarily responsible for supporting effective, seamless delivery of integrated care to seniors in the sub-region that SASCO Home is the Integrated Community Care Provider (ICCP). The role requires fostering collaboration between Senior Care Centres (SCC), Active Ageing Centres (AAC), Home Personal Care (HPC+), and Home Therapy (HT) Services of different partners in the sub-region to meet seniors' diverse needs. The manager will track and report on the performance of the service delivery in the sub-region.

Duties and responsibilities

Coordination Between Partners:

  • Facilitate collaboration among SCC, AAC, HPC+, and HT providers for smooth ICCP service delivery.
  • Facilitate ICCP partners’ leadership meetings to set strategic directions, address service gaps, and assess resource opportunities.
  • Facilitate ICCP partners’ operational level meetings to plan and manage the successful delivery of the 4 core eldercare services in the sub-region.

Reporting & Administrative:

  • Oversee timely submission of progress reports and claims to MOH and AIC.
  • Act as the main point of contact for ICCP-related administrative matters.

Additional Responsibilities:

  • Undertake other duties as required to ensure the ICCP function’s success.
  • Proactively identify challenges and opportunities for service improvement.


Qualifications & Skills Required

  • Bachelor’s degree in healthcare management, gerontology, or a related field.
  • 4 – 6 years of community care experience.
  • Familiarity with senior care services and community care delivery models.
  • Strong understanding of performance metrics, KPIs, and regulations in healthcare.
  • Strong leadership, communication, and interpersonal skills.
  • Strategic thinking aligned with operational priorities.
  • Proactive, solution-oriented mindset with focus on results.
