Job Responsibilities
Social Worker Associate
- Conduct one-on-one counselling sessions with new residents and/or their Next of Kin (NOK) prior to admission, providing a comprehensive overview of the nursing home’s policies, services, and daily routines.
- Address any concerns, questions, or fears the resident or NOK may have, ensuring that they feel informed and supported before transitioning into the nursing home.
- Monitor the adjustment process for new residents during their first days or weeks in the nursing home, ensuring they feel welcomed and comfortable.
- Provide emotional support to residents and NOKs to ease any anxiety or uncertainty about the transition.
- Conduct a thorough Bio-Psycho-Social-Spiritual (BPSS) assessment for each resident. Assess the severity of risks and protective factors to guide intervention planning.
- Gather insights into their needs, preferences, and any challenges they face. Include family members or significant others in the process where possible.
- Work closely with healthcare professionals (e.g., nurses, doctors, therapists) and other staff members to gather holistic information about the resident’s condition and overall care needs.
Resident’s Engagement
- Understanding of each resident's unique interests, preferences, and hobbies to ensure that engagement activities are tailored to their needs and desires
- Plan and organize a variety of activities that reflect the interests, cultural backgrounds, and cognitive abilities of residents
- Actively motivate residents to join in activities through personal invitations and group encouragement. Be respectful of their individual comfort levels.
- Create an environment that fosters positive interactions and a sense of belonging, making residents feel comfortable and motivated to participate
- Work closely with CC and volunteers to ensure activities are effectively planned and executed, catering to the diverse needs of residents.
- Assist in the training of volunteers, ensuring they understand their roles in supporting residents during activities, and are equipped to provide meaningful engagement.
Others
- Initiate ACP awareness with residents and their NOKs
- Participate in internal and/or external committee/workgroup activities as well as research work as directed by Senior MSW/Lead of PSS Services
- Cover the duties of the other staff (MSW, MSWA) in his/her absence.
