- Screen all new patient admissions and identify patient with complex care needs.
- Participates in clinical rounds with the multidisciplinary team (MDT) and raises issues on complex care and social issues.
- Collaborate with the primary team for integrated care interventions, resource availability for discharge planning.

- Function as a resource person and expert to the ward nurses and case managers.
- Participate in policy reviewing activities that influence case management practices and the health of the community.
- Collaborate with other healthcare team members and participates in Regional Health System (RHS) programmes and events e.g. Community Health Screening, managing frequent admitters.
- Develop, conduct and monitor quality performance indicators for continuous improvement.
- Diploma in Nursing or State Registered Nurse Certificate with relevant Advanced Nursing Diploma or equivalent
- At least 5-8 years of clinical experience
- Good interpersonal and communication skills
