Job Description:Social Worker (MHCS)JOB DESCRIPTION:CASEWORK AND COUNSELLING
  • Manage case referrals.
  • Manage 30 C&C active cases (mainly Group 2 cases) per quarter.
  • Conduct assessment, planning and intervention for cases.
  • Conduct case conferences with partners.
  • Participate in networking and case management meetings.
  • Document and evaluate case outcomes.
  • Develop and review appropriate intervention strategies.
  • Ensure reports on cases are submitted in a timely manner
INFORMATION AND TRIAGE
  • Refer clients to appropriate agencies for further intervention.
  • Part of the Intake & Triage Team to conduct intake assessments for new intakes based on the practice standards of the Code of Social Work Practice and Practice Guide.
COMMUNITY WORK
  • Network with other organizations and stakeholders.
  • Community Needs Assessment.
JOB REQUIREMENTS:
  • Degree or Graduate Diploma in Social Work
  • Social Work education and understanding of social work values and ethics
  • Understanding of work with vulnerable populations i.e. low-income families
