
Coordinator/ Associate - Social work
Responsibilities:
- Conduct and facilitate Advance Care Planning (General ACP) discussions for clients.
- Ensure that clients’ preferences and decisions are clearly documented.
- Maintain accurate and up-to-date records of ACP discussions, decisions, and documentation.
- Ensure compliance with legal, regulatory, and industry best practices.
- Oversee locum ACP facilitators' scheduling and workload.
- Provide administrative support for locum incentives and payments.
- Assist in recruitment, onboarding, and offboarding processes of locum facilitators.
- Plan and conduct ACP awareness talks for the public.
- Organize and participate in networking events, workshops, and seminars to promote ACP awareness.
- Support reporting officers in any other ACP-related tasks.
- Prepare reports and updates on ACP program progress.
Requirements:
- Possess a Diploma in Social Work or equivalent.
- Trained as a facilitator in Advance Care Planning (ACP General)
- At least 2 years of related experience
