  • Assist in financial assessment and applications for financial schemes under the supervision of Medical Social Workers (MSWs).
  • Conduct initial assessments and provide basic case management support.
  • Perform information and referral functions to connect clients with appropriate services.
  • Conduct home visits independently or jointly with MSWs or the Home Care team when required.
  • Provide administrative support for the department’s daily operations.
  • Maintain accurate records and documentation of client cases and financial assistance applications.
  • Work closely with a multi-disciplinary team to support clients’ needs.
  • Support project and program development to enhance service delivery.
  • Attend training sessions and casework supervision for continuous learning and improvement.
  • Perform any other related tasks as directed by reporting officers.

Requirements:

  • Possess a Diploma in Social Work or equivalent.
  • Experience in means testing and healthcare setting is advantageous
  • At least 2 years of related experience
