  1. Make sure the webpage is open.
  2. In the Safari app on your Mac, choose Safari > Preferences, then click Security.
  3. Make sure the Enable JavaScript checkbox is selected.
  4. Click Websites.
  5. On the left, click Content Blockers.
  6. Make sure Off is chosen in the pop-up menu next to the website.
  7. On the left, click Pop-up Windows.
  8. Make sure Allow is chosen in the pop-up menu next to the website.

Responsibilities

Establish and maintain relationships with internal care teams and external stakeholders, including patients and caregivers
Support preventive health screenings, reduce readmissions, and address social determinants of health to improve patient outcomes
Utilize technology to enhance patient health literacy and access to healthcare resources
Assist in coordinating and delivering patient care, including tracking and documenting clinical indicators using IT systems
Conduct basic health assessments, close preventive screening and vaccination gaps per national programs
Provide basic health education on lifestyle management, social engagement, and health monitoring, including guidance on vaccinations and home-use health devices

Requirements

Experience in healthcare sector is a plus!
Strong communication and interpersonal skills
Strong team-player, with natural ability to interact with clients and/ or families
