DESIGNATION : Senior Social Worker/Case Worker
RESPONSIBILITIES
The Senior Social Worker provide comprehensive social services and support to individuals and their families. This role involves assessing the social, emotional, and practical needs of seniors, developing care plans, and coordinating services to enhance the well-being and quality of life for the clients. The Senior Social Worker will work collaboratively with healthcare teams, community organizations, and families to ensure holistic care and support for seniors in various care settings. The Senior Social Worker also provide supervisions to junior staff in managing casework and other related support work to the clients.
Casework Management
- Conduct assessments of the clients and caregivers to determine their social, emotional, health and physical needs. Review case notes and documents
- Develop and implement individualised care plans and interventions
- Provide holistic and integrated care for clients and caregivers through basic emotional support, monitoring, service linkage and conducting meaningful activities
- Prepare case recordings and social reports
- Evaluate progress and effectiveness of casework interventions
- Conduct closure of cases upon achieving sustainable service outcomes for clients, families and communities
Stakeholder Engagement and Partnerships
- Build professional relationships with clients, significant others and other professionals
- Participate in outreach to community partners to strengthen support and care coordination to address service gaps and service delivery issues
- Evaluate progress and effectiveness of the collaboration efforts for the programme
Programme and Staff Management
- Support in managing the social service programme / service, including monitoring the progress and performance
- Provide supervision and guidance to junior staff in the team and look into their professional development needs
QUALIFICATIONS
- Bachelor's Degree in Social Work, Counselling or a related field of study from an accredited institution
OTHER INFORMATION
Relevant Experience
- At least 3 - 5 years of casework experiences
- Accredited as Registered Social Worker with Singapore Association of Social Workers would be an advantage
