JOB PURPOSE

Are you a seasoned Nursing professional with a passion for leading and innovating community-based services? Thye Hua Kwan Moral Charities is seeking an Assistant Director (Clinical) to oversee and elevate our Home Based Services, a key pillar under our Seniors' Community Services.

Our Seniors' Community Services provide a seamless continuum of care that supports seniors in staying engaged, living independently, and receiving the right care at the right time. Home Based Services play a vital role in this ecosystem, ensuring that seniors who require medical care, daily assistance, or specialized support can continue to live safely and comfortably at home.

In this pivotal leadership role, you will drive strategic initiatives, ensure clinical excellence, and enhance the quality of life for seniors in our community.

DUTIES & RESPONSIBILITIES

Clinical & Service Excellence

  • Ensure clinical governance, quality assurance, and risk management frameworks are in place.
  • Strengthen care coordination pathways between Home Based Services, Hospitals, and Community Care providers.
  • Drive continuous quality improvements through data-driven decision-making, outcome tracking, and service evaluations.
  • Implement person-centred care models that address the medical, social, and mental health needs of seniors.

Operations & Resource Optimization

  • Monitor and optimize service efficiency, resource allocation, and financial sustainability.
  • Develop and enforce standard operating procedures to ensure high service standards.
  • Leverage technology and digital solutions to improve care delivery.
  • Secure funding opportunities and manage budgets to support service sustainability.

Leadership & Strategic Planning

  • Oversee the strategic planning, development, and operations of Home-Based Services.
  • Ensure Services align with national healthcare regulations, policies, and best practices.
  • Develop and implement evidence-based care models to enhance service delivery.
  • Identify gaps in eldercare services and drive the expansion and innovation of Home-Based Programs.

Team Development & Workforce Capability Building

  • Lead, mentor, and develop multidisciplinary teams across various functions within Home Based Services, ensuring a high-performing and collaborative workforce.
  • Establish succession planning, training programs, and leadership pipelines to build a future-ready workforce.
  • Promote staff well-being and engagement, fostering a collaborative and mission-driven culture.

Stakeholder & Partnership Engagement

  • Build and strengthen partnerships with MOH, AIC, Hospitals, Community organizations, and regulatory bodies.
  • Represent and participate in national workgroups and forums, advocating for improvements in home-based eldercare.
  • Work with Community partners to enhance care integration and support networks for seniors.
QUALIFICATIONS
  • Registered Nurse (RN) with a valid Singapore Nursing Board (SNB) license.
  • Bachelor's degree in nursing; a master's degree in healthcare management, Gerontology, Public Health, or a related field would be advantageous.
  • Minimum 10 years of clinical and healthcare leadership experience, with at least 5 years in a senior management role, ideally overseeing home-based care, community care, or eldercare services.
  • Proven leadership in managing and developing multidisciplinary teams across various home-based care functions.
  • Expertise in clinical governance, nursing quality assurance, and regulatory compliance, ensuring alignment with MOH, AIC, and SNB standards.
  • Strategic planning and operational management experience, with a strong track record in developing, scaling, and enhancing home-based or community healthcare services.
  • Strong stakeholder engagement and partnership-building skills, with experience collaborating with government agencies, healthcare institutions, and community organizations.
  • Financial and resource management experience, including budgeting, funding models, and service sustainability strategies.
  • Strong analytical, decision-making, and communication skills, with the ability to drive nursing innovations, service improvements, and policy development.
