DESIGNATION : Social Work Associate
RESPONSIBILITIES
- Facilitate the completion of Advance Care Plans (General ACP) for clients.
- Maintain comprehensive and up-to-date records of advance care planning discussions, decisions, and documentation, ensuring compliance with industry best practices, legal and regulatory requirements.
- Oversee locum ACP facilitators' scheduling and workload and provide the necessary administrative support for incentives payment.
- Assist in the recruitment, onboarding and offboarding process of locums.
- Lead and participate in networking and outreach community events, workshops, and seminars to promote awareness and understanding of advance care planning.
- Plan and conduct advance care planning awareness talks for the public.
- Any other related duties as directed by the said reporting officers.
QUALIFICATIONS
- Possess a Diploma in Social Work or equivalent.
- Trained as a facilitator in Advance Care Planning (ACP General).
- Proficient in local languages and fluent in at least one local dialect.
- Excellent communication and interpersonal skills, with the ability to connect effectively with partners and clients.
- Strong coordinating and facilitation skills
- Able to work independently and collaboratively in an interdisciplinary team environment.
- Demonstrate empathy and sensitivity to the emotional and psychological aspects of advance planning discussion.
