  1. Make sure the webpage is open.

  6. Make sure Off is chosen in the pop-up menu next to the website.
  7. On the left, click Pop-up Windows.
  8. Make sure Allow is chosen in the pop-up menu next to the website.

DESIGNATION : Head

RESPONSIBILITIES
  • Leading, planning and development of social work practice standards, quality outcomes, in support of HNF's overall mission to empower patients, clients and caregivers in the community through all-round support.
  • Responsible for overseeing the planning, development, and delivery of social work and mental health services, in line with HNF's overall mission and care model.
  • Ensure quality and cost-effective delivery of social work and mental health services in home care, caregiver support and community outreach.
  • Identify needs and develop appropriate social work services in support of HNF patients, caregivers and community.
  • Review, develop and enhance work processes/guidelines of social work department and CREST team to ensure efficiency and compliance with government guidelines and regulations, such as Means Testing, MediFund and ILTC subsidy scheme
  • Guide, manage and develop Social Work Department's staff to meet the department's objectives, including managing workload distribution, developing training plans and providing supervision and mentorship to staff in the department.
  • Collaborate with internal clinical and non-clinical colleagues in clinical care and in support of HNF's goals.
  • Identify and develop appropriate networking and collaborative partnerships with external organizations to advance HNF's mission.
  • Any other duties assigned by the said reporting officer.


QUALIFICATIONS

  • Bachelor of Arts degree in Social Work or its equivalent. A Certificate in Counselling would be an advantage

  • Registered Social Worker

  • At least 8 to 10 years of social work experience, with at least 3 years of supervisory experience, and 1 year in healthcare

  • Able to apply knowledge and experience to address practical problems of staff and clients
