- Assist in formulation and revision of guidelines, standards and procedures.
- Participate in research studies - including data management and data analysis.
- Participate in quality improvement projects where appropriate.
- Work independently within the guidance established by the service protocol, policies and practices for the care, comfort and safety of the patients.
- Function as a clinical resource to all members of the healthcare team in the assessment of complex patient care needs and situations and in the determination of interventions.
- Co-facilitate, co-organise the various support and therapy groups, workshops, and other activities planned by the department/service.
- Assist in proposing, redesigning and implementing strategies to improve clinical services.
- Participate in relevant IT developments, workgroup meetings and research, if required.
- Provide and coordinate continuity of care from initial consultation through to recovery.
- Act as pivotal team member for patient care by informing the multidisciplinary care management team of patient updates and initiate case review and discussion.
- Work with the patient and family, and various agencies/professionals in both the hospital setting and/or within the patient's community.
- Provide proactive problem-solving techniques in response to needs assessment, such as initiating appropriate referral and liaison with other professional, agencies and/or step-down facilities.
- Formulate and develop care management plans for patients in collaboration with clinicians and other health care members, including initiating and coordinating management planning for patients.
- Monitor and evaluate patient's response to treatment and revising treatment/care plan as needed.
- Establish a thorough working knowledge of resources and information for dissemination to patient and family.
- Assist in the collection and input of data for clinical audit of service.
- Provide counselling and supportive therapy for patient and family in response to their needs. This includes therapeutic work in home settings with attending social worker and team colleagues where necessary.
- Provide psychoeducation to patient and family.
- Co-facilitate patient education, support and therapeutic groups.
Job Requirement:
- Degree in Psychology/Social Work/Counselling or relevant healthcare experience in psychiatry.
- At least 2 years of relevant working experience or the equivalent.
- Good Team player who possesses good communication and strong interpersonal skills.
