Social Work Case Manager

THKMC is the charitable arm of Thye Hua Kwan Moral Society, which lives by the mission To Serve Mankind to help anyone who needs help regardless of their race, religion, colour, language, creed and culture. THKMC today serves more than 70,000 beneficiaries through more than 70 programmes across five Services, which include Disability, Early Intervention for Children, Family, Seniors' Community, and Therapy.
RESPONSIBILITIES
We are seeking a compassionate and organised Social Work Case Manager to assess client needs, coordinate services, and advocate for individuals and families. The ideal candidate will possess strong communication skills, problem-solving abilities, and a deep commitment to helping others.
Key Responsibilities:

Conduct client assessments and develop individualised service plans.

Connect clients with resources (housing, healthcare, financial assistance, mental health services, etc.).

Advocate for clients' rights and coordinate with service providers.

Maintain accurate case records and ensure confidentiality.

Monitor client progress and adjust service plans as needed.

Work collaboratively with community organizations, healthcare providers, and government agencies.

Provide crisis intervention and emotional support when necessary.

Stay updated on social work policies, programs, and ethical standards.
- Bachelor's Degree in Social Work or a related field (psychology, sociology, counselling, human services)
- At least 1-3 years of experience in case management, social services, or related field
- Strong communication, organisational skills, problem-solving, and advocacy
