Job Purpose
The incumbent will be responsible to provide casework support to the Therapeutic Group Home (TGH) Clinical team for residents residing in TGH.
Responsibilities
- Support TGH Social Worker in areas of case management and counselling of boys admitted under Care and Protection Orders addressing their emotional, behavioural and developmental needs to ensure their safety and well-being.
- Facilitate admission procedures for boys admitted to Boys’ Town, which includes handling intake interviews and liaising with stakeholders working with the boys prior to admission.
- Assist with development and implementation of Individualised Care Plan (ICP) for all residents admitted. As part of this ICP, you will be required to work closely with other internal and external stakeholders to ensure holistic care for the residents.
- Supporting the Social Worker in engaging clients and families.
- Assisting with Medical Appointments for clients.
- Assisting with Supervising Family Access.
- Assist in conducting psychoeducational and therapeutic group work programmes as advised by unit lead.
- Support in liaising with external agencies e.g., Child Protection Officers, Probation Officers, Social Workers, Family Service Centres, Police for case management purposes and to support the case plans for the residents.
- Work in collaboration with a multidisciplinary team, including social worker, psychologist, youth specialists and unit leads to ensure residents needs are met.
- Maintain proper documentation for case files, updating case notes in a timely manner.
Requirements
- Diploma in Social Work.
- Able to commit to a 1-year temporary contract.
- Good interpersonal and communication skills.
- Understand the impact of residential care and able to preempt and address the developmental needs of children and young persons.
