Job Description:Case Management Aspects
  • Organise and co-conduct intake interviews with clients and caregivers
  • Collect information and conduct assessments of clients
  • Coordinate, implement and follow up on individual care plans for clients
  • Collaborate with Case Intervention Team to provide support and intervention to clients
  • Contribute to case reviews
  • Handle case related administrative duties and documentation
  • Adhere to the standard operating procedures and case work practices
Crisis Management Aspects:
  • Handle crisis situations, such as behavioural challenges in collaboration with relevant professionals
  • Mediate conflicts between clients, caregivers, and staff when necessary
Programme Management Aspects:
  • Work with team to design, implement, facilitate and review training programmes to equip clients with pre-vocational and vocational skills
  • Network and collaborate with relevant stakeholders and community partners
  • Maintain and compile statistics for reporting, funding or audit purposes
Qualifications:REQUIREMENTS
  • Diploma/Degree in Psychology, Counselling, Sociology or a related field
  • Knowledge and experience in working with special needs or disabilities will be an advantage
  • Good listening and communication skills to work with clients with diverse abilities and backgrounds
  • Good interpersonal skills to liaise with stakeholders and external agencies
  • Strong organisational and planning skills
  • Passionate and committed to working with people with disabilities
  • Good team player with a positive and resilient attitude
  • Proficient in Microsoft Office and administrative skills
  • Able to work on some weekends or weekday evenings
