JOB SUMMARYYou will be expected to be independent in carrying out continuous patient care, be proactively participating in quality improvement and research projects going on within TTSH Department of Respiratory and Critical Care Medicine (RCCM). You will also participate in coordinating any programme or events carried out in the department.You will serve as a programme coordinator for the Chronic Obstructive Pulmonary Disease (COPD) post-exacerbation pathway programme. This role is essential in optimising care for patients transitioning from hospital to home after an acute exacerbation of COPD. The staff will be responsible for patient case coordination and management, project administration, and ensuring a seamless transition to prevent re-admissions and reduce hospital length of stay.Training will be provided to the staff to ensure he/she is well-equipped to support patients effectively.Target Population: Patients discharged from TTSH after an inpatient admission for an acute exacerbation of COPD, with approximately 500 admissions annually. You will work closely with the Clinical Lead and Project Team to recruit and manage these patients who fall under NHG's River of Life's 'Crisis & Complex Care' segment.MAIN DUTIES AND RESPONSIBILITIESService Quality (10%)
  • Assist in monitoring and tracking key performance indicators (KPIs).
  • Maintain timely and accurate database records of patients in the Programme.
  • Facilitate communication and collaboration with stakeholders to maintain project alignment and address any issues promptly.
  • Uphold requirements of governing documents (where required) including standard operating procedures and work instructions.
Clinical Patient Care/Coordination (80%)
  • Recruit subjects, covering screening, informed consent-taking and managing enquiries from patients.
  • Conduct phone reviews with patients at stipulated touchpoints to assess their symptoms, review patient self-management goals and provide COPD education.
  • Assess patients' COPD symptoms and discuss their interests in Advance Care Planning (ACP), making referrals for ACP discussions as needed.
  • Assist in the arrangement of home visits (if required) for Allied Health to visit patients at homes.
General (10%)
  • Manage and handle all programme administration, including claims and reporting, maintaining effective documentation and reports, and tracking timelines.
  • Liaise with Clinical Lead and/or Programme Team to plan and coordinate multidisciplinary meetings, ensuring follow-through on agreed actions.
  • Perform any ad-hoc tasks as assigned by the supervisor or Programme Lead.
Job Requirements:
  • Mandatory nursing background.
  • Experience in caring for respiratory patients is an added advantage.
  • At least 2-4 years of industry experience in an acute public hospital or healthcare institution.
  • Good collaborative skills and ability to work with multiple stakeholders (clinical healthcare professionals, finance etc.) across different domains.
  • Organised, meticulous individual with good project management skills.
  • Ability to multi-task and work under tight deadlines in a fast-paced environment.
  • Dedicated and willing to go the extra mile to see things through.
  • Excellent written, oral communication presentation skills (English).
  • Proficiency in Microsoft Office (Word, Excel, Powerpoint).
