Community Dietitian
The Community Dietitian shall work within our Foundation's community setting to promote healthy eating and improve nutritional well-being.
Key Responsibilities:
- Nutritional Assessment and Counseling:
- Assess the dietary needs of referred individuals and groups in our Medical Division, including clients from our Family Medicine Clinic, Dialysis Centre, Rehab services, Food Farmacy and Active Ageing Centres.
- Provide personalized nutrition counseling to referred clients to help manage their chronic conditions (e.g., diabetes, heart disease, obesity).
- As much as possible, offer plant-based dietary advice based on cultural, social, and economic factors that influence food choices.
- Health Promotion and Education:
- Develop and implement nutrition education programs and workshops for e.g. Active Ageing Centres, Food Farmacy, Dialysis clients.
- Collaborate with other Social Service Agencies and community partners to design nutrition outreach programs.
- Co-create educational materials with relevant teams and stakeholders (e.g., brochures, posters, and social media content) to promote whole-food plant-based healthy eating.
- Collaboration and Teamwork:
- Collaborate with healthcare professionals such as doctors, nurses, and social workers to ensure comprehensive care for individuals.
- Participate in multidisciplinary teams to provide integrated health and nutrition services.
- Monitoring and Evaluation:
- Oversee monitoring and tracking of clients participating in nutrition programs e.g. 21-day Challenge
- Evaluate the effectiveness of community nutrition programs and suggest improvements based on data and feedback.
Requirements:
- Education: Bachelor's degree in Dietetics
- Certification: Accredited Dietitian of Singapore (ADS) with Singapore Nutrition and Dietitics Association
- Experience: Experience working in community health, public health nutrition, or a clinical setting. Experience working with diverse populations and addressing social determinants of health is beneficial.
- Skills:
- Strong communication and interpersonal skills.
- Ability to present nutrition information to a variety of audiences.
- Knowledge of nutrition-related software and tools for assessments and record-keeping.
- Ability to work independently and in a team environment.
