2. Job Description
- Provide casework and counselling services to clients and their families
- Assess needs of clients and their families and formulate Individual Service Plans (ISPs)
- Provide Information & Referral (I&R) to link clients and their families to appropriate services
- Ensure that the Halfway House’s Programme meets the mission, objectives and procedures of case management services
- Provide a proper filing system for Case Management and ensure relevant documents are filed accordingly, in accordance with audit requirements.
- Assist in the preparation of new casefiles (with relevant documents)
- Assist in compilation of statistics and preparation of routine reports, e.g. monthly reports, etc.
- Formulate and guide residents through case management for all residents assigned under his care.
- Track and evaluate the residents’ progress during their stay at the Halfway House. This includes involvement in case discussions among the relevant Halfway House Department and case conferences with the partner Singapore Prison Service officers.
- Manage residents’ case files and ensure that all case information and relevant documents are complete and updated regularly.
- Compile relevant details and provide timely updates (e.g. residents’ progress updates and attendance for sessions/activities and events etc.) for reporting purposes.
- Provide case management support as well as plan, review and conduct rehabilitative programs for the residents.
- Render professional social work services and advice to clients and their families.
- Identify, plan initiate and monitor existing and new programs so as to enhance the standard of social work services in the Halfway House.
- Work closely with external agencies such as the Ministry of Home Affairs (MHA), Singapore Prison Services and other social welfare organizations.
- Perform direct casework by doing initial intake interview, case analysis, treatment planning, family mediation and counselling.
- Assist in planning and implementing existing and new programme
- Liaise and coordinate with volunteers for programs and activities
- Involve/assist in PERTAPIS activities
- Any other duties as assigned by the Head of Home/Deputy Head of Home
3. Job Requirements
- Degree in Social Work, Psychology, Counselling OR Accredited Social Worker OR Accredited Social Service Practitioner
- Minimum 2 years of relevant working experience
- Knowledge of basic IT (MS Word, Excel, Access and Powerpoint) and basic statistical analysis
- Good interpersonal and communication (oral and written) skills
