The Challenges

You will be part of a dynamic team to champion patient and caregiver education efforts within hospitals and communities. This includes developing and implementing cluster-wide, hospital and community projects and initiatives to enhance patient, caregiver and resident activation. You must work collaboratively with health and social care stakeholders to support nation-wide initiatives such as Ministry of Health’s Healthier SG and build relationship-based care with the wider community of partners, patients, caregivers, volunteers and residents.

Your Key Responsibilities Include, But Are Not Limited To

  • Lead in the development, management and execution of community enabling and empowering platforms, projects and initiatives
  • Design and develop patient education resources, platforms, and programmes to build a community of carers who are equipped with the skills, knowledge and confidence to take ownership of their health
  • Support clinical leads in research and ground sensing projects relating to health activation, health literacy and patient empowerment
  • Facilitate focus group discussions and collate insights to identify opportunities for actionable change
  • Build and facilitate communities of practice made up of health and social care professionals, patients, caregivers and volunteers
  • Co-lead in the conceptualisation and execution of hospital and community events and regular community dialogue sessions with patients, caregivers, residents and partners
  • Drive publicity campaigns to raise awareness of department's key initiatives
  • Manage department resources and implement programmes, projects and events within set budgets, funding mechanisms and timeline
  • Provide secretariat support at key stakeholders' meetings

The Requirements

  • Strong passion for population health
  • Degree in Social Sciences/Behavioral Science/Psychology/Social Anthropology or a related field
  • At least 2-3 years of corporate experience, preferably in the healthcare and/or social care sectors
  • A creative thinker with excellent written and verbal communications skills
  • Excellent team leader and player with a positive can-do attitude
  • Ability to multi-task, resourceful and highly resilient
  • Work effectively under tight deadlines and manage projects independently
  • Excellent knowledge of social media platforms and website management
  • Experience in event management and vendor management
