Care@Community Department aims to deliver holistic care that is centred on an individual’s needs. It mainly targets seniors that are frail, whom have multiple care needs, that will become increasing complex with time. With the integration of community-based aged and social care services, it enables the seniors to continue to stay in the community for as long as possible, therefore allowing them to age-in-place.

Responsibilities

  • Assist in providing Advance Care Planning Facilitation to the AAC Seniors.
  • Interact with AAC seniors and link them up with Palliative Care services.
  • Organize Workshops to build capabilities within amongst staff.
  • Organize Activities such as quarterly awareness talks and quarterly day trips.
  • Organize Activities that are tailored towards client’s needs.
  • General Admin Duties.

Requirements

  • Minimum GCE ‘A’ Level, Diploma
  • Good communication & interpersonal skills
  • Proficiency in English and Mandarin is essential as this role will need to organise English & Chinese workshop and activities
  • Good public speaking skills
  • This position is on contract basis.
